Some of our common forms are linked below. Please know that once you are a client, you will have access to a client portal with all necessary forms required for engagement in our programs.
Release of Information
A release of information (ROI) is a legal document that authorizes the sharing of a person's private information while ensuring compliance with privacy laws. We are only able to communicate with others related to your care when there is a valid ROI on file. You have the right to revoke the ROI at anytime.
Grievance Form
A grievance form is a document used to formally record and submit a complaint or concern, providing details needed for review and resolution. We take your feedback seriously and ask that you inform us of your concerns at your earliest convenience. You can download the form and send via email to info@cjsw.org, mail it to us, or drop it off at our office. You can also submit the form online, just click the 'Submit' button.
